Missed Appointment/Late Cancellations/Late Rescheduling

• Cost of appointment is due for missed appointments or late cancellations and rescheduling.

• Appointments missed, cancelled late or rescheduled late take away the opportunity from another person to schedule in your time slot.

• Cancellations and rescheduling must be made at least 24 hours in advance.

Phone Use Fees

• Urgent Care Phone Consultation: $30 + $60 for 30-min appointment or $120 for 1-hour appointment.

• There are no fees for phone calls regarding new patient inquiries, appointment scheduling, billing, questions to clarify treatment.

Email Use Fees

• There are no fees for emails regarding new patient inquiries, appointment scheduling, billing, questions to clarify treatment.

• Emails that require medical advice will be charged $10-30 and/or an appointment will be recommended.

• I answer non-urgent emails as time allows. If you have an urgent matter, call the office phone and leave a message. The doctor will call you as soon as she’s able.

• E-mail is not a confidential source. If you want to communicate in a confidential manner, please schedule a time to talk with Dr. White using the HIPAA compliant telemedicine computer platform.
