Phone Use Fees
Pre-scheduled Phone Appointment: Care for Chronic Conditions and Acute Care fees apply (see above).
Acute/Urgent Care Phone Consultation: $30 mininum charge; $2 per minute will be charged after the first 5 minutes.

Free call-in time for current treatment is Mon-Fri 12-1pm, except for official holidays. Outside these hours, every phone call to the acute/urgent care phone is charged.
Costs for Questions about Current Treatment

There is no charge for simple questions about current treatment. Please e-mail or call the office. The doctor will return your call when she is available.

There is no charge to provide the doctor updates about your condition. This information is welcomed and appreciated. Please provide updates by e-mail or office voice-mail.
Other Fees
Missed Appointment/Late Cancellations/Late Rescheduling: Cost of appointment missed

Appointments missed, cancelled late or rescheduled late take away the opportunity from another person to schedule in your time slot. Cancellations and rescheduling must be made at least 24 hours in advance.
Reduced Fee Opening: 50% of fees

There are two openings for reduced fee. These openings are usually filled continuously and turn-over-time varies. If you would like to be put on the wait list for reduced fee, please contact the office.

Criteria for keeping the reduced fee: one appointment must be made at least every 3 months. If this is not done, the patient is bumped from the reduced fee slot and will be put on the wait-list if desired.
